Job Description:
Seeking a PRN Nurse Practitioner for home risk assessments,
welcoming new graduates.
Conduct comprehensive adult/geriatric assessments in patients'
homes, skilled facilities, and community settings.
Review health history, medication adherence, social factors, and
identify risks to improve care and close gaps.
Collaborate with physicians, case managers, and caregivers; provide
patient education and follow-up.
Flexible hours including days, evenings, nights, and weekends;
travel required within Bloomingdale, OH.
Earn $1,900-$9,500/month based on assessments completed; mileage
reimbursement provided.
Qualifications include current RN and NP licensure, certification,
1+ year experience, strong communication, and computer skills.
Travel necessary; valid driver--------s license and insurance
required.
