Program implementation is accomplished by application of best practices and by using standardized program requirements, procedures and/or curriculum. Provides outreach and education to providers and community partners. Also assists with COVID 19 response activities as assigned. Other duties performed include: Provides service navigation support and care coordination. Selects, develops and/or modifies educational strategies/curriculum, intervention methodologies, and approach to meet the unique needs of clients or students. Develops and maintains partnerships with local partners for networking and coordination of care for clients, reduce risk factors, and improve outcomes for clients served. Utilizes generally established and common statewide practices; and may develop educational curriculum and materials. Identifies and addresses individual risk factors through screening, assessment and education with emphasis on changing behavior and improving the health outcomes for those participating in the program. Provides, encourages, and directs individuals towards behavior change and performs care coordination for those participating in various programs. Provides technical assistance to community groups and programs to ensure the successful implementation of funded programs. Considers opportunities for process improvement, and implements improvements within the scope of work. Changes to job duties and requirements may occur. Compensation $5,305.29 to $7,109.58 per month for a 172.67 hour work month. Excellent County benefit package when eligible. This union-represented position is available immediately. This is a limited-duration employment position as defined by Deschutes County Code Chapter 3.08.010. The position may be terminated by Deschutes County based upon the availability of discrete revenue sources. If the specified discreet revenue source has yet to terminate, the position may be terminated by the County to fulfill its business needs, as determined by the sole discretion of the County. This position will terminate no later than December 31, 2021. Should funding changes occur such that the position is no longer restricted by the specified discreet revenue source, the County at its sole discretion may at that later date offer regular employment status to the incumbent in this position. This position is open until filled. Applications will be reviewed on a weekly basis. Minimum Qualifications Requires at least a Bachelor's degree in a health or education field and three years of public health experience; OR a Master's degree in a health or related field and at least one year of public health experience. Preference will be given for: Master's degree in a health or related field and at least twoyears of public health experience. Bilingual in English and Spanish. Must have knowledge of: Public health and infectious disease. Health communication and education. Must have a demonstrated ability to: Act in a timely, effective and efficient manner to collect information from a variety of sources, consult with experts, document thoroughly and provide education about public health principles and recommendations. Take initiative to determine gaps in meeting objectives and troubleshoot accordingly. Organize, gather and produce relevant information. Establish and maintain effective working relationships with the public, other employees, and community partners at all times. Act professionally with regard to code of ethics and confidentiality. Communicate effectively both verbally and in writing. Work independently as well as in a team setting. Necessary Special Requirements Possession of or ability to obtain a valid Oregon driver's license within 30 days of hire date. The employment offer will be contingent upon presentation of an acceptable and verifiable driver's license, pre-employment screening for criminal history, driving history, and controlled substances (NOTE: Positive test results for marijuana use may result in rescission of a contingent offer of employment). This screening must be completed with satisfactory findings in order for a formal offer of employment to be extended. Must have dependable transportation and be willing and able to drive throughout the tri-county area in sometimes adverse weather conditions. Must be willing to work a flexible schedule when necessary, to meet client and department needs. Physical Requirements. Must have the ability to work in an office that is environmentally controlled; possess sufficient hand coordination to perform keyboarding and data entry; arm/hand movements to retrieve work materials and operate a variety of general office equipment. The position is frequently required to sit, walk, stoop, bend, kneel, reach, stand, grasp, talk, hear, see, and may occasionally lift up to 10 pounds. Excellent County benefit package when eligible.
