Job Title - Occupational Therapist Department - PACE Reports to - PACE Team Director Benefits - Medical, Dental, Vision, Life Insurance (Short & Long Term Disability) 403(b) Retirement Plan Paid Holidays PTO CME (Continuing Medical Education) Eligibility for Public Service Loan Forgiveness About Position: Work Location: 1214 Vaughn Rd, Burlington, NC 27217 Schedule: Monday through Friday, 8:00am to 5:00pm Duties/ Responsibilities - The Occupational Therapist (OT) is responsible for performing timely assessments of participants, and for developing therapy plans and plans of care based on an individualized assessment. The OT participates a member of the interdisciplinary team, assesses for appropriate DME, and assists with 24 hour care coordination. Qualifications - Education/Experience : Graduate of an Occupational Therapist curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; be eligible for the National Registration Examination of the American Occupational Therapy Association and have achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Pubic Health Services. 1 year experience as an Occupational Therapist and 1 year of experience with the frail or elderly preferred. Ability to demonstrate clinical competency with client-centered services to geriatric population Must be able to work under pressure Must meet a standardized set of competencies for the specific position description established by PHSC and approved by CMS before working independently. Excellent teamwork and collaboration philosophy and skill Licensure, Registry or Certification Required : Licensed Occupational Therapist in NC. Valid NC driver’s license and vehicle. Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
