a qualified Hospice RN in Asheboro, North Carolina, 27203! Job
Title: Hospice RN Profession: Nursing Specialty: Hospice Duration:
13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 5:00 PM
Experience: Minimum of one (1) year experience as an RN, with a
focus on hospice, home health, oncology, long-term care, or acute
care hospital settings. License: Current license to practice
nursing in the state of employment Certifications: Current BLS
certification from the American Heart Association; current
automobile insurance; valid driver’s license Must-Have: - Knowledge
of terminally ill patients and their families, as well as an
understanding of hospice principles. - Excellent patient assessment
skills. - Good oral and written communication skills. - Ability to
document and manage programs effectively. - Knowledge of general
nursing practices and regulatory guidelines. - Ability to operate
various nursing assessment tools and technology. Description: The
Registered Nurse (RN) will provide direct patient care services to
hospice patients. The RN will also provide supportive care to the
patient's family. The position involves managing an
interdisciplinary group consisting of support staff to enhance the
quality of hospice care. The RN is responsible for case managing
all assigned patient clinical records. Assures the highest quality
of care is provided and documented in accordance with the Plan of
Care. Performs initial and ongoing assessments of the patient.
Helps develop and implement the Hospice Plan of Care as prescribed
by the physician and in conjunction with other team members. Works
closely with other members of the interdisciplinary team to provide
comprehensive services including physical, psychosocial,
educational, emotional, and spiritual support. Reports any change
in the patient's condition to the attending physician. Maintains
accurate and updated medical records, including all
interdisciplinary notes and assessments. Practices infection
control measures and standard precautions. Monitors scheduling of
all staff involved in patient care. Participates in the Quality
Assessment and Performance Improvement Program. May be requested to
be involved in on-call schedules. Promotes the company's core
values. Completes required compliance training annually. Performs
other relevant activities as assigned. Attends staff meetings to
discuss patient care and organizational matters. Maintains
knowledge of pain and symptom management in the dying process.
Conducts routine supervisory visits with all team members at least
monthly. Performs ongoing reviews of patient clinical records to
assure quality of services. Maintains close contact with the
patient's family to provide information and support. Acts as the
primary liaison between physicians, patients, and hospice teams.
Responsible for educating patients, families, or other caregivers
regarding needs identified in the Plan of Care. Supervises Aides
and prepares written assignments for them. Ensures home visits are
made at least every two weeks to assess care and services provided.
