ASAP About the Position Key Responsibilities: Coordinate discharge
planning and patient needs assessments Manage patient plans of care
and DME orders Ensure compliance with CMS (Centers for Medicare &
Medicaid Services) standards Collaborate with interdisciplinary
teams for smooth transitions of care Requirements ? Minimum 2 years
of recent RN experience ? Acute hospital background ? Experience
with ICU, MS, PICU, SDU, PCU, IMC, and Observation units ? Previous
Charge Nurse experience ? Strong Case Management & Utilization
Review skills ? BLS Certification ? EPIC EMR proficiency
