RN Case Manager Location: Danville, VA Duration: 13 weeks
Guaranteed hours: 36 Hours Shift: (9A - 5:30P) - Every Other
27413448EXPTEMP Job Requirements Must Have Skills/Experience:
Discharge planning in the acute hospital setting. Preferred
Skillset/Experience: ACM or CCM preferred. Creating discharge
plans, based on insurance benefit, knowledgeable of utilization
reviews, coordinating services to prevent readmission.
Knowledgeable of CMS regulation and Virginia Medicaid Common
Diagnosis/Treatment: Cerebral Vascular Accident, Diabetes, Heart
and Vascular, Sepsis Required Degree Compact RN License or VA BLS -
